The correct form(s) must be prepared and submitted for filing at the Court Office. Details are available in the Civil Proceedings Rules 1998, as amended and the Family Proceedings Rules 1998, as amended. The prescribed forms are available in the appendix to the Civil Proceedings Rules 1998 and the Family Proceedings Rules forms in the schedule to the practice direction dated March 16th 2004.
